These applications are required in order to meet the various requirements mandated by the Texas Department of State Health Services (DSHS), local healthcare facilities, and other regulatory agencies for college healthcare students that enter the patient care setting as part of their program. View a summary of the HPI Application Process. Each application will list the requirements for admission, but in general all will include:

New immunization Form! After 12/1/15, this single page form will be the only one accepted.

Some vaccinations (Hepatitis B in particular) can take up to 6 months to complete the vaccination process, so prior planning is critical. For a listing of currently required immunizations, please visit this page. Some immunization requirements may differ between healthcare training programs. The program application for the individual program will specify which immunizations are required for that program.

Tuberculosis Questionnaire

Some Health Professions courses require the submission of the following form:

Criminal Background Check

For program applications submitted to the Health Sciences Department (EVC) and Human Services Department (RGC) only, the student will be instructed when to initiate the background check AFTER they have submitted their program application for admissions. For program applications submitted to the Health Professions Institute (HBC), students must initiate the background check process only when instructed to do so. Your acceptance to the program is conditional upon successful completion of the criminal background check. Background Checks will not be accepted if they were completed more than 3 months before the start date of the program/course. The background checks are conducted by CastleBranch, a firm specializing in background checks for healthcare workers.

Drug Screening

Incoming students should initiate a drug screen ONLY upon directions from the department.

Program Specific Requirements

Each program may have additional application requirements (i.e. proof of current Healthcare Provider CPR; high school diploma or GED, etc). Additional information, and downloadable applications are available in the above applications (PDF Files). Students may also contact the appropriate Coordinator for each program for additional information or assistance.
